Hi,
My Orion Smart states 11.9V all,of the time regardless whether the van is running, lights on etc.
status is always Off and
charge is disabled due to: input voltage lock out, engine shutdown detected.
does anyone know why the input voltage never changes even when driving etc. ?

What do you measure on the input? Maybe alternator fuse blown?
It wont start charging because minimal is set to 12,4.
1. Or the voltage is realy 11.9 volt.
2. Or the orion would be defective.
But i would say option 1.
Normally the starter battery is charged with a charging voltage of approximately 14.4 volts. In Euro 5/6 cars, however, the charging voltage can drop to as much as 11.5 volts due to the above mechanism.
I always make sure to connect the D+ signal to the orion to force charging with euro 5/6 otherwise you get these kind of issues

On the Victron Connect app the input voltage is ALWAYS 11.9 regardless of driving.
I changed the lockout voltage down to 11.5 and nothing happens at all - it doesn't change anything.
Personally I think this unit is faulty - it used to work fine and it stopped.

Perhaps the vehicle's alternator is faulty. To rule that out, with the engine running, use a voltmeter to measure the voltage at the starting battery terminals. As mentioned it should be around 14.4 volts.
This is a software issue on the unit. To fix this, one meeds to disconnet both the starter and lithium batteries. This will hard reset the unit. Mine was stuck at exactly the same voltage, 11.91 V to be exact. Engine runnnin the actual input was 14.34 V. Hopefullly this helps to find the bug. This is also discussed here:

Ps. It would also be nice if some features were added. The solar charger is much smarter. At least it would be nice to see the charging current. Having similar history to MPPT would be even better.
